Checklist of Needed Documentation for Veterans Benefits

VA Aid & Attendance Documentation Checklist

- Original DD-214 military discharge form q Social Security card or number
- Marriage certificate(s)
- Death certificate of Spouse
- Death certificate of Veteran
- Proof of in-home healthcare costs including care provider information (name and phone number)
- Proof of facility costs, such as a copy of a recent bill
- Proof of monthly health insurance premiums
- Current bank statement from ALL accounts (all pages)
- Current financial statement from brokerage accounts, annuities, IRA accounts, etc.
- Proof of all fixed income sources, such as a SS award letter and pension 1099’s, Etc.
- Approximate home value and lot size
- Legal documents (will, living will, POA’s)
- Most current year federal tax return
- Information regarding current and all previous marriages. Must include the city/state of marriage, divorce or death
Documents for the Surviving Spouse
- Social Security card or number
- Maiden name (copy of birth certificate)
- Information regarding current and all previous marriages. Must include the city/state of marriage, divorce or death
